The cornerstone of Search and Care's work is our compassionate and comprehensive care for our vulnerable and at-risk elderly neighbors in Manhattan's Yorkville, Carnegie Hill, and East Harlem neighborhoods. Our model is unique in that we emphasize care management - not case management, maintaining professional relationships with our clients that delve deeper in order to address not only their urgent needs, but emerging and future ones as well. In a sense, we act as their surrogate family, providing concrete help such as obtaining benefits and entitlements (i.e. Meals on Wheels, EISEP homecare, Food Stamps), organizing home care and transportation (i.e. Access-a-Ride), managing finances (e.g. paying bills, budgeting, securing stipends), addressing mental health needs (e.g. providing emphatic listening, connections to specialized resources and referrals, therapeutic groups), providing nursing support (e.g. monitoring nutrition, medication and vital signs, home safety/comfort, advocating with doctors and nursing facilities, preventing falls), offering volunteer assistance (e.g. companionship, socialization, shopping, medical escorts), and pet care (e.g. upkeep, visiting).
